The Centers for Medicare & Medicaid Services (CMS) created the Five-Star Quality Rating System to help consumers, their families, and caregivers compare skilled nursing centers more easily and to help identify areas they may want to ask questions about. The 5-star quality rating system isn’t a substitute for visiting the skilled nursing centers. Use the 5-star ratings together with other sources of information.
Heritage Care Center to serve as a ‘peer coach’ for other Nebraska nursing homes.
To be eligible to serve as a peer coach, a nursing home must be identified as being in the top ten percent of homes; as reported by the National Nursing Home Quality Measure Composite Score provided by the Centers for Medicare & Medicaid Services (CMS).
Heritage Care Center will also partner with their regional Quality Innovation Network.
Network-Quality Improvement Organization (QIN-QIO). CIMRO of Nebraska, the Kansas Foundation for Medical Care, Quality Health Associates of North Dakota and South Dakota Foundation for Medical Care make up the Great Plains Quality Innovation Network, the QIN-QIO for the region. Heritage Care Center agreed to partner with Great Plains Quality Innovation Network as part of the Great Plains Quality Care Collaborative, an initiative to bolster quality improvement in nursing homes across the state.
